PETITION U/S 13 (3) (B) BIRTH AND DEATH REGISTRATION ACT.

In the above noted case, the petitioner begs to state as follows.

1.      That, the petitioner is the resident of the place mentioned above within the Jurisdiction of this Honourable Court.

2.      That, the petitioner's father namely (fathers name) s/o- (grandfathers name) (father) & (grandmothers name) (mother) died on dated (date of death) at his residential house village ………….., PO – …………, P.S,- ………… Dist.- ……….,(State Name).

3.      That, owing to inadvertence and some unavoidable circumstances the petitioner could not be able to register the date of death of his father namely (fathers name) s/o- (grandfathers name) before the Registrar Birth and Death as required under registration of Birth and Death.

4.      That, now the petitioner being living with his family and legal heirs is in need of Death Certificate of petitioner’s father namely (fathers name) s/o- (grandfathers name).

5.      That, the petitioner is ready and willing to abide by any term and condition to avail the Death certificate of his father.

It is therefore, prayed that your Honour may be pleased to condone the delay for no registration of the date of death of the of the petitioner within the period stipulated under the Act. and pass an order directing the Registrar of Birth and death i.e (Concerned Authority) to register the date of death of the petitioner’s father after making proper inquiry for which Act of kindness the petitioner shall be duty bound and ever prayed.
