1.    That, I have applied before the authority of TP WESTERM ODISHA DISTRIBUTION LIMITED for supply of power to the premises pertaining to plot no ............. under khata no .......... area of A.............. D At Mouza- ................, Tahasil ................... P.S. Saintala in the district of .................. (State)

2.    That the ROR of holding no ............. of mouza .................., PS no ............ PS – ...................., is stand recorded in the name of my grandfather namely  Late ........................., s/o - Late ................................... .

3.    That, after the death of grandfather namely Late ....................u in an amicable oral family partition the aforesaid land fallen into my share, since then I am in peaceful possession over the aforesaid land and it is free from any encumbrances & litigation.

4.    That, I have no outstanding dues payable to the present distribution licensee/ TP WESTERM ODISHA DISTRIBUTION LIMITED or any other licensee/supplier (TPNODL, TPSODL and TPCODL) within the state of Odisha.

5.    That, there is no outstanding dues payable to any of the distribution licensee/supplier within the state of Odisha by myself or my spouse, parents, children of any of the family members relating to the same premises or any other premises.

6.    That, there is no arrears of electricity dues or other dues payable to any of the distribution licensee/supplier within the state of Odisha by the Company firm or Trust with which I am associated either as a partner director or trustee.

7.    That, there is no arrears of electricity dues or other dues pending unrealised against the premises to which supply power has been sought for and applied before the TP WESTERM ODISHA DISTRIBUTION LIMITED.

8.    That, this affidavit is sworn by me for the purpose of submission before the authority of TP WESTERM ODISHA DISTRIBUTION LIMITED for supply of power to the premises mentioned in foregoing para.

9.    That, no body has got any objection if I shall utilize the same in any manner.

10.                That, the fact stated in paras (1) to (9) are true to the best of my knowledge and belief.
