            I, Sri Ramesh Nag, aged about 34 years, son of Dharani Nag, Village: Belgaon, PO: Belgaon, P.S.: Saintala in the district of Balangir at present  Village: Belgaon, PO: Belgaon, P.S.: Saintala in the district of Balangir by profession Daily wages labour do hereby solemnly affirm as follows:-

1.    That my income from all sources is ₹22.000/- (Rupees Twenty-Two Thousand) only

2.    That I or any of my family members do not own or possess any residential house/building or homestead land in the jurisdiction of rural area.

3.    That, I own or posses a residential house / building of homestead in the jurisdiction of town but this has inadequate accommodation the extent of the homestead land being __________________ cents.

4.    That, my family consist of beside myself.
