3.       That, on dt. .............................. at about .............................. while I was going from my home to our cultivating land the said mobile phone along with the sim card lost from my possession.

4.       That, despite diligent efforts I have been unable to trace the lost phone with sim card.

5.       That, this affidavit is made for obtaining a duplicate sim card from the service provider and for any other legal purpose.
