AFFIDAVIT

I, Lakshman Bag aged about 41 years, s/o – Bishnu Bag, r/o village Dabjor (Bhirkapada) Po – Karamtala, PS – Sainata do hereby solemnly affirm and declare as follows:-

1.    That, I am the deponent of this affidavit and permanent resident of the place mentioned above.

2.    That, my daughter namely Siddhi Bag is a student of class VII of Govt. Girls High School, Saintala.

3.    That, her real name is Siddhi Bag which is in her Aadhar Card bearing no. 784713379810 and her name has been wrongly enrolled as Siddhirani Bag in school admission register instead of Siddhi Bag.

4.    That, Siddhi Bag and Siddhirani Bag is same and one person, except her no other person is in her village.

5.    That, as a guardian I want to change my daughter’s name as Siddhi bag instead of Siddhirani Bag in school admission register of Govt. Girls High School, Saintala.

6.    That, I am swearing this affidavit to file it before the concerned authority to change my daughter name as Siddhi Bag instead of Siddhirani Bag in school admission register and for future reference.
